This application must be accompanied by an application fee of $13,299.00 (Incl GST).
Most attachments are already included in the application form. Links to the separate forms are provided below in case you require additional copies.
Attachment A - Attachment cover sheet
This is to be completed for each attachment to the Exporter Accreditation Application Form.
Attachment B – Statutory Declaration for section 3.8
This is to be completed by the Chairman or appropriate executive officer to attest that material risks facing the applicant have been identified and that the applicant has sufficient risk management systems in place that are current, accurate and are being actively managed.
Attachment C - Summary and assessment of each executive officer
This is to be completed by the applicant for each executive officer and submitted as part of an application for accreditation.
Attachment D - Checklist for each executive officer
This checklist must be completed and signed by each executive officer and submitted as part of an application for accreditation. For further details see section 7.2 of the Exporter Accreditation Application Form.
Attachment F - Facility Cover Sheet
This is to be completed for each finance facility, intercompany loan, committed and uncommitted line of credit and off balance sheet finance available, or currently being negotiated by the applicant, to carry out the export proposal described in section 4 of the Exporter Accreditation Application Form.
Additional executive officers
Under section 7.4 of the Exporter Accreditation Application Form, details of each executive officer must be provided. The Application Form provides space for three executive officers. This form can be used if an exporter has more than three executive officers.
